Return the completed referral, along with the patient's Mental Health Treatment Plan, using either of the options below.
How the referral process works

Complete the form
Fill in the referral form with your patient's details, presenting concerns and any relevant clinical background.

Attach supporting documents
Include the Mental Health Treatment Plan and referral letter so your patient can access their Medicare rebate.

Send it through
Email or call our reception team. We'll be in touch with your patient to arrange their first appointment.
Referrer information
Our Clinical Psychologists provide evidence-based assessment and treatment for adults, adolescents and children. We accept referrals under Medicare's Better Access initiative, the Chronic Disease Management Scheme, DVA and private health.
